The main game plays exactly the same as previous entries in the series. The only changes are the updated 3D graphics and the ability to see the next three pills that will be dropped into the jar. The two music tracks introduced in Dr. Mario 64 have also been renamed from "Cube" and "Que Que" to "Cough" and "Sneeze".
Dr. Mario Online Rx multiplayer mode is for two players only, but it can be played online through the Nintendo Wi-Fi network. You are also given the option to play with Dr. Mario or as one of the Mii characters saved to the Wii.
Between one and four players can take part. Using the Wii Remote, players have to pull Megavitamins into place to clear the viruses on the screen. You can set the virus level and music for this mode.
Virus Buster is based on a minigame originally from Brain Age 2 on the Nintendo DS.
